NOTE: Please be aware an online telemedicine intake assessment with provider, and questionnaire completion is required prior to obtaining services. Optional in clinic free initial 15 minute consultation. (Over 15 minutes, there is an additional charge). During telemedicine or office visit, will discuss lab work requirements. If labs have been obtained prior within 30 days, please submit prior to our visit. If labs or additional labs are required, provider will provide laboratory order. Once all lab work is completed, an in-person visit is then scheduled to discuss your treatment options and individualize your care.

  • Human Chorionic Gonadotropin (HCG) Injections  (If Applicable)     (Additional charge per month) 

  • PDE5 Inhibitors (If Applicable) Assist with Erectile Dysfunction 

  • Hormone Enhancing Prescription Given 

  • Vitamin B 12 Injections Available (Lab Dependent) 

  • Selective Estrogen Receptor Modulator (SERM)- Increase Testosterone and Sperm production If Fertility is Desired (Additional charge per month) 

  •  Unlimited Follow Up Visits During Treatment Plan Regarding Male Hormone Replacement Therapy. 

  • If Wishing For Provider To Manage Other Comorbidities /Diagnosis, There Is An  Additional Charge.
